“Development
of a Geriatric Pharmacy Practice Model in Transitional Care” |

Description: |
The
aims of this project are to create, design, and evaluate a novel
transitional care geriatric pharmacy practice model ideal for
generating scholarship and providing a stimulating environment
for teaching pharmacy students and other learners about geriatric
pharmacotherapy and geriatric pharmacy practice. Medication reconciliation
and optimization of drug therapy regimens in seniors transitioning
from hospital back to home can be complicated, and there is very
little pharmacist involvement in this process at most institutions.
At VCU MC, we have a program managed by nurse practitioners that
helps transition our patients at highest risk for poor short-term
outcomes back to the community, which has reduced costs and re-hospitalizations
over the last 6 years. Medication-related problems are common
in these patients. This project details a specific plan for my
professional development as a clinician, teacher, and researcher
to facilitate creation, implementation, evaluation and dissemination
of an innovative pharmacy practice and education model. |
